This study examines its impact on patient outcomes and care coordination using a mixed methods design with both quantitative and qualitative evaluations. The research draws on publicly available data and performance reports from leading healthcare institutions such as Kaiser Permanente, Johns Hopkins Medicine, and the Mayo Clinic.
